The H2020 funded research project ValueCare has concluded after 5 years of creation, innovation and cooperation. What is left behind, the legacy of the project, is manyfold. A new model of healthcare delivery based on the value that is created for the patients and supported by ITC in 7 pilots and older people with chronic conditions, a training programm on shared decision making, a technical solution composed of an app, a virtual coach and a platform for professionals as well as methodologie for co – desingning innovative activities with patients. The journey is not finishing here…this is just the beginning.
