On October the 16th, 2021, Mr. Anastasios Rentoumis, the president of AfIC will present online the H2020 project, VALUECARE, taking place at Trieste Citizen Accelerator, SESSION B “Experiences of connection with the territory for the solution of the phenomena of isolation: the integrated network – The role of ICT in active and healthy ageing”, at 11:10 a.m. CET.
In this event organized by the Technology Park Pordenone, located in the Friuli Venezia-Giulia of Italy, AfIC will present how the adoption by Health Care Systems, of a model of care provision based on Patient Reported Outcomes and Experiences (PROMS and PREMS) can have a positive impact on the sustainability of health care systems in Europe, especially in relation to seniors that have multiple chronic conditions like Diabetes and Hypertension.
