Would’ it be more fair for health care users, health care professionals and the Health System itself, to implement integrated health care service reimbursement models , based on the improvement of outcomes that matter most to patients, such as quality of life, self efficacy in self management of chronic conditions and so on?
In the current edition of BMC Geriatrics, the H2020 “ValueCare” project research team, presents the study protocol of implementing such a model in 6 European countries, using supportive ICT’s. In Greece and the Athens Pilot, AfIC, together with the clinical team of Athens Medical Center, we are implementing this model, using a pre – post, controlled design, with end users being independent living patients suffering from T2DM and Hypertension.
